A full-time Primary Care Physician is sought to support a growing eldercare program that emphasizes community-based care, whole-person treatment, and a collaborative interdisciplinary approach. This position focuses on serving medically complex senior patients through comprehensive in-person assessments, care coordination, and oversight of medical treatment across settings including homes, nursing facilities, and hospitals.
Conduct in-person comprehensive assessments for new and existing patients.
Coordinate participant care plans in collaboration with a full Interdisciplinary Team (IDT).
Oversee care for patients admitted to nursing homes and hospitals (including short-term, long-term, and acute placements).
Monitor and manage transitions of care (e.g., admits and discharges).
Guide and supervise Nurse Practitioners and other team members as needed.
Visit participants in nursing homes as needed, based on individual care plans.
Participate in rotating night and weekend call coverage.
Support seamless coordination with community physicians and other specialists.
Doctorate in Medicine (MD or DO) from an ACGME-accredited residency program
Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred)
Active California Medical License (unrestricted)
Active DEA license and state-controlled substance registration
Eligible for enrollment as a provider in Medicare and Medi-Cal
Experience working in geriatrics, palliative care, or home-based medicine
Strong collaborative, patient-centered care philosophy
Skilled in interdisciplinary care delivery models
Management experience required
No travel required
