We are seeking a quality-driven Primary Care Physician (MD/DO) to join our post-acute care team in Farmington Hills, MI. This is an in-person, full-time opportunity (5 days/week) rounding in skilled nursing facilities.This role offers a rare blend of strong clinical autonomy, dedicated operational support, and high earning potential. Key Responsibilities

• Perform comprehensive patient assessments, medical histories, and physical exams.
• Develop, implement, and adjust individualized long-term care plans.
• Monitor clinical progress and coordinate care to optimize post-acute patient outcomes.
• Collaborate closely with facility leadership, nursing staff, and multidisciplinary teams.
• Educate patients and families on health management, disease progression, and preventive strategies.
• Maintain accurate, timely medical records in full compliance with healthcare regulatory standards.

Requirements & Qualifications

• Licensure: Active Physician (MD or DO) license in the State of Michigan.
• Credentials: Valid DEA registration.
• Experience: Background in post-acute, acute, emergency department, or clinic settings preferred.
• New Grads: Open to recent residency graduates with a strong clinical foundation.
• Skills: Strong clinical acumen, excellent communication skills, and a collaborative leadership approach.
• Location: Ability to commute regularly to facilities in Farmington Hills, MI (Required).
