    Healthcare United is seeking an experienced Physician Assistant or Nurse Practitioner for a full-time outpatient primary care float opportunity based in Shelton, Connecticut.

    This role requires at least five years of provider experience. The ideal candidate has strong outpatient primary care experience and can work confidently across clinic needs.

    The incoming provider will evaluate and manage patients, deliver preventive care, treat acute and chronic conditions, support clinic coverage, and collaborate with physicians and clinical staff.

    Responsibilities

    • Provide outpatient primary care services
    • Float as needed to support patient access and clinic coverage
    • Evaluate and manage adult or family medicine patients
    • Conduct wellness and sick visits, provide preventive care, and manage chronic conditions
    • Develop treatment plans and coordinate follow-up care
    • Collaborate with physicians, APPs, nurses, and clinical staff
    • Maintain accurate EMR documentation


    Requirements

    • Certified Physician Assistant or Nurse Practitioner
    • Active or eligible Connecticut APRN or PA license
    • Minimum of five years of provider experience required
    • Primary care, internal medicine, family medicine, urgent care, or outpatient experience preferred
    • Strong independent clinical judgment
    • Excellent communication and documentation skills
