Job Description:

We are seeking a quality-driven Primary Care Physician (MD/DO) to join our post-acute care team in Adrian, MI. This is an in-person, full-time opportunity (5 days/week) rounding in skilled nursing facilities.This role balances exceptional clinical autonomy with robust operational support. You will deliver high-quality care to a diverse patient population, lead an experienced multidisciplinary team, and build lasting, meaningful relationships with patients and their families.Compensation & Benefits

• Earnings: $165,000 - $261,000 estimated annual base.

Key Responsibilities

• Perform comprehensive patient assessments, medical histories, and physical exams.
• Develop, implement, and adjust individualized patient care plans.
• Monitor clinical progress and coordinate care to optimize post-acute outcomes.
• Collaborate closely with facility leadership, nursing staff, and multidisciplinary teams.
• Educate patients and families on health management and preventive strategies.
• Maintain accurate, timely medical records in full compliance with regulatory standards.

Requirements & Qualifications

• Licensure: Active Physician (MD or DO) license in the State of Michigan.
• Credentials: Valid DEA registration.
• Experience: Background in post-acute, acute, emergency department, or clinic settings preferred.
• New Grads: Open to recent residency graduates with a strong clinical foundation.
• Skills: Strong clinical acumen, excellent communication skills, and a collaborative leadership mindset.
• Location: Ability to commute regularly to facilities in Adrian, MI (Required).
