Job Description: Primary Care Home Care RN

As part of a person centered care model, this role will provide skilled nursing care to clients in home and community based settings ensuring the delivery of outcome based, high quality, cost effective care. This role will collaborate with all team members to achieve

Qualifications

Licensure: Registered Nurse with a license to practice in the State of Connecticut

Education: Graduate of an Associate''s or diploma program in nursing; graduate of NLN approved BSN program, preferred

Experience: One year nursing experience or one year nursing experience in a specialty; medical/surgery or home care, preferred. Computer skills preferred.

Standard Hours Per Week: 40

Full-time (40 hours)

Shift Details: Float position for Watertown office coverage area. Mon -Friday 8-4:30. Coverage one weekend per month, 1 Major and 1 minor holiday per year.
