    Description

    Healthcare United is seeking an experienced Nurse Practitioner or Physician Assistant for an outpatient primary care float opportunity based in Shelton, Connecticut.

    This role requires at least five years of provider experience and is ideal for an APP with strong outpatient primary care experience and the ability to work confidently across clinic needs.

    The incoming provider will evaluate and manage patients, provide preventive care, manage acute and chronic conditions, support clinic coverage, and collaborate closely with physicians and clinical staff.

    Responsibilities

    • Provide outpatient primary care services
    • Float as needed to support patient access and clinic coverage
    • Evaluate and manage adult or family medicine patients
    • Conduct wellness visits, sick visits, preventive care, and chronic disease management
    • Develop treatment plans and coordinate follow-up care
    • Collaborate with physicians, APPs, nurses, and clinical staff
    • Maintain accurate EMR documentation
    • Support continuity of care and patient satisfaction


    Requirements

    • Certified Nurse Practitioner or Physician Assistant
    • Active or eligible Connecticut APRN or PA license
    • Minimum of five years of provider experience required
    • Primary care, internal medicine, family medicine, urgent care, or outpatient experience preferred
    • Strong independent clinical judgment
    • Excellent communication and documentation skills
