Purpose of this position: The Primary Care Director is the administrative leader responsible for achieving a cost competitive, value driven system of care for the areas assigned to the clinical departments/units/ or service areas (areas of responsibility).
Primary functional responsibilities include:
Provide administrative and operational leadership for the areas of accountability
Develop and execute the long range, strategic growth plan for areas of accountability considering population health and value based care
Develop and implement care models/systems that coordinate care for seamless transitions across the continuum
Collaboratively establish standards of care that drive programs, clinical, functional and financial outcomes and patient and staff satisfaction
