Post-Discharge Coordinator (local remote)

Primary Duties and Responsibilities:

Join Tandigm as a Post-Discharge Coordinator and help members feel supported as they transition home after a hospital stay. In this role, you'll connect with members after discharge, complete post-discharge assessments, and help them schedule and complete important follow-up visits with their primary care provider. You'll also identify care coordination needs and connect members to Tandigm's Social Work, Pharmacy, or Nursing teams when additional support is needed. Essential Functions

• Consistently exhibits behavior and communication skills that demonstrate Tandigm's commitment to superior customer service, including quality, care, and concern for all internal and external customers.
• Conducts telephonic outreach to post-discharge patients within 24-48 hours of transitioning from a post-acute setting. As appropriate, serves as a resource for patients about follow-up appointments, coordination support and other logistical needs for as long as the patient receives post-discharge services from Tandigm.
• Schedules follow-up appointments for patients with PCPs, as needed, per their clinical state after transitioning out of a post-acute environment.
• Referring to pharmacists, social work or nursing as needed, to ensure patients' needs are being adequately addressed.
• Engages patients through telephonic outreach to promote positive patient outcomes through coordination with Tandigm's care team and/or PCP.
• Maintains up-to-date knowledge and awareness of the services and procedures of clinical programs.
• Delights patients and providers with a customer-service mindset, responding to questions mindfully, providing insightful information, and ensuring patients and providers are left with a positive view of Tandigm.

Required Qualifications:

Education

• High School Diploma or GED is required; bachelor's degree preferred
• Medical Assistance certificate preferred

Experience

• Experience with transitions of care calls
• Post discharge support
• Patient engagement

Knowledge, Skills, Abilities

• Ability to organize and coordinate required follow-up appointments for patients in a seamless fashion.
• Engaging and positive personality, with strong communication skills that enable effective communication with patients during initial outreach.
• Comfortable with medical terminology and familiar with a variety of relevant clinical conditions.
• Attention to detail and ability to multi-task in order to capture details from outreach efforts and seamlessly handle handoff member lists to the Clinical Coordinator.
• Language skills to support multilingual needs across the member base.
