The Care Coordinator works in collaboration with providers, clinic nurses, practice managers, and other healthcare professionals to develop, implement, evaluate, and revise the delivery of care coordination services to those patients and patient populations determined to be at high risk or in need of specific care/quality-related services. The care coordinator is responsible for collaborating directly with providers, community agencies, and acute-care staff/providers to create consistent and coordinated delivery of high-quality healthcare services to targeted populations.
