The Care Coordination Aide supports CCP's Population Health and Concierge Care Coordination team by helping members connect with the programs, services, and resources they need. This is a great opportunity for someone who enjoys working with people, solving problems, and making a difference in the healthcare experience.
In this role, you will support care managers and the care coordination team by communicating with members and providers, assisting with referrals and outreach, gathering information, coordinating services, and keeping records accurate and up to date. You will also help with department projects, reporting, member education, and other activities that support quality care and member engagement.
