In this role, you will help ensure people we support receive high-quality, person-centered services that promote independence, community inclusion, and achievement of their personal goals.Planning Coordinator DutiesThe Planning Coordinator is responsible for ensuring regulatory and agency compliance while supporting people through person-centered planning and service coordination. Responsibilities include monitoring and maintaining Life Plans, ensuring accurate and timely documentation, overseeing program budgets and individual funds in accordance with agency procedures, and advocating for the people supported. This role promotes community integration and develops positive relationships with families, service providers, employers, and community partners while upholding Mozaic's mission, vision, and values. The Planning Coordinator also safeguards the privacy, dignity, and confidentiality of people receiving services.Planning Coordinator SkillsThis position requires strong organizational skills, attention to detail, and the ability to manage multiple priorities independently. The successful candidate will demonstrate professionalism, strong communication and teamwork skills, sound judgment, and discretion when handling confidential information. Proficiency with computers and office technology is required. A valid New York State Driver's License is required, including the ability to operate agency vehicles and wheelchair-accessible vans.
