    Primary Care - Darby, part of Mercy Fitzgerald Hospital, is seeking a Board-Certified/Board-Eligible Family Medicine or Internal Medicine Physician to support continued primary care expansion and access initiatives across the Pennsylvania market.

    The Practice

    • Established, high-demand primary care practice
    • Immediate patient volume available
    • 100% outpatient - no inpatient responsibilities
    • Team-based care model (APP, support staff)
    • Integrated Epic EMR
    • Strong referral network within Trinity Health Mid-Atlantic Medical Group
    • Specialized care support including Endocrinology

    The Opportunity

    • Complex Chronic Disease Management: Diagnose and manage advanced, often co-occurring chronic conditions like poorly controlled diabetes, severe hypertension, substance use disorders, and asthma exacerbated by urban environmental factors.
    • Social Determinants of Health (SDOH) Intervention: Screen for and address non-medical barriers to health, including food insecurity, housing instability, transportation deficits, and low health literacy.
    • Trauma-Informed Care: Utilize trauma-informed communication strategies to care for patients experiencing chronic stress, chronic illness, or historical medical mistrust.
    • Interdisciplinary Care Coordination: Collaborate closely with on-site social workers, case managers, community health workers, and behavioral health specialists to provide holistic patient support.
    • Preventive Medicine & Harm Reduction: Administer critical immunizations, perform routine health screenings, and implement harm reduction strategies for high-risk behaviors.
    • Resource Optimization: Navigate financial barriers by utilizing sliding fee scales, prescribing low-cost formulary medications, and leveraging local public health assistance programs.

    • Full-time outpatient schedule (36 patient-facing hours)
    • Flexible scheduling options available
    • Minimal call responsibilities (1:5 call schedule), phone-only
