Opportunity Information:

Sutter Medical Group is seeking a Post‑Acute & Geriatric Medicine Physician to join our physician‑led post‑acute care team in Sacramento, CA.

This role is ideal for physicians who enjoy caring for medically complex patients during the critical transition from hospital to recovery, and who value autonomy, continuity, and long‑term patient impact.

You will serve as the primary physician of record for patients in skilled nursing and rehabilitation settings, managing complex medical conditions, leading interdisciplinary care, and helping patients recover, stabilize, and avoid unnecessary hospital readmissions. This is a high‑impact, longitudinal practice with strong clinical ownership and a sustainable lifestyle.

This position is an excellent fit for:

  • Geriatric Medicine fellows seeking a focused, high‑impact clinical practice
  • Internal Medicine residents or fellows with an interest in post‑acute care, complex chronic disease, or transitions of care
  • Physicians who enjoy autonomy, clinical decision‑making, and continuity
  • Hospitalists looking for a long‑term, lifestyle‑sustainable practice outside of traditional inpatient schedules
  • Clinicians interested in value‑based care, quality improvement, and population health

What Makes This Practice Different:

  • Physician‑led, integrated medical group with strong hospital partnerships
  • Focus on clinical decision‑making and outcomes, not volume‑driven care
  • Emphasis on continuity after hospitalization and preventing avoidable readmissions
  • Structured rounding model with a manageable patient census
  • Collaborative, interdisciplinary team environment
  • Opportunities to participate in quality improvement, education, and system‑level initiatives

Key Responsibilities

  • Serve as the primary physician for patients in skilled nursing and rehabilitation facilities
  • Manage medically complex post‑acute and long‑term care patients, including multi‑morbidity and polypharmacy
  • Conduct scheduled and responsive rounds to assess recovery, respond to acute changes, and adjust care plans
  • Collaborate closely with nursing, rehabilitation, case management, and social work teams
  • Communicate with patients, families, and caregivers regarding goals of care and treatment plans
  • Partner with hospitalists, primary care physicians, and specialists to ensure smooth transitions of care
  • Participate in quality improvement, population health, and value‑based care initiatives
  • Maintain timely and accurate documentation in the electronic medical record
Qualifications:
  • Medical degree (MD or DO) from an accredited school of medicine.
  • Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred).
  • Active and unrestricted medical license in California.
  • Demonstrated experience in post-acute, skilled nursing, long-term care, or geriatric settings.
  • Strong clinical skills in managing complex, multi-morbid patients.
  • Excellent communication and interpersonal skills, with a patient- and team-centered approach.
  • Ability to work effectively in an interdisciplinary, facility-based environment.
  • Proficiency in electronic medical records (EMR) systems.

Preferred Qualifications:

  • Fellowship training in Geriatric Medicine or experience in post-acute/rehabilitation medicine.
  • Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics.
  • Experience participating in value-based care or population health programs.
Join Us and Enjoy:
  • Employed position with predictable schedule and strong work‑life balance
  • Sign‑on bonus and relocation assistance
  • PSLF‑eligible position
  • Zero‑cost medical insurance for employees
  • Paid medical benefits for dependents
  • 100% employer‑funded 401(k) match plus profit sharing
  • CME allowance
  • Practice autonomy within a supportive physician‑led group
