We are seeking a skilled and compassionate Physician to join Legacy Care on a part‑time basis in Richmond, VA. This role involves providing high‑quality primary and acute medical care within post‑acute and long‑term care facilities, working collaboratively with multidisciplinary teams to ensure safe, person‑centered care, timely transitions for residents, and ongoing support for facility staff.

Conduct comprehensive assessments and develop individualized care plans for residents in post‑acute and long‑term care settings
Provide diagnosis, treatment, medication management and follow‑up care consistent with current evidence‑based practice
Respond to acute changes in condition, coordinate timely interventions and, when appropriate, facilitate transfers of care
Collaborate with nursing staff, advanced practice clinicians, therapists, social workers and facility leadership to promote holistic, person‑centered care
Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
Support medication safety, antimicrobial stewardship and quality improvement initiatives
Provide clinical consultation on complex cases and mentor facility staff and medical colleagues as needed
Medical degree (MD or DO) from an accredited institution
Active and unrestricted Virginia medical license (or eligibility for licensure)
Board certification or board eligibility in Family Medicine, Internal Medicine or related specialty
Clinical experience in post‑acute care, long‑term care, geriatrics or related settings preferred
Proficiency with Electronic Medical Record systems and clinical documentation
Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
