Location: Stony Brook Community Medical, PC – Centereach, NY Position Type: Full-Time Position Summary Stony Brook Community Medical, PC is seeking a compassionate, board-certified or board-eligible Palliative Care Physician to join our multidisciplinary team in Centereach, New York. The physician will provide comprehensive, patient-centered palliative care services to patients with serious, chronic, and life-limiting illnesses. This role focuses on improving quality of life by addressing physical symptoms, emotional well-being, psychosocial needs, and goals of care while working collaboratively with patients, families, caregivers, and healthcare providers. Key Responsibilities - Provide comprehensive palliative care consultations and ongoing management in outpatient, inpatient, skilled nursing, and/or home-based settings as assigned.
- Evaluate, diagnose, and manage pain and other complex symptoms associated with serious illness.
- Conduct goals-of-care discussions, advance care planning, and end-of-life care conversations with patients and families.
- Collaborate with primary care physicians, specialists, nurses, social workers, case managers, and other interdisciplinary team members to develop individualized care plans.
- Coordinate transitions of care across healthcare settings.
- Educate patients, families, and caregivers regarding disease progression, treatment options, and symptom management.
- Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR).
- Participate in quality improvement initiatives and interdisciplinary team meetings.
- Provide consultation and education to healthcare staff regarding palliative care best practices.
