    Description

    We are seeking a dedicated and compassionate Physician to join our clinical team, providing high-quality care within several Skilled Nursing Facilities (SNFs). This role is ideal for a clinician who enjoys building long-term relationships with patients and collaborating with multidisciplinary teams to manage complex medical needs.

    While experience in long-term care is preferred, we are committed to training the right candidate.

    Key Responsibilities

    • Clinical Care: Provide comprehensive medical management for patients in SNF settings, focusing on internal medicine, family practice, endocrinology, and critical care stabilization.
    • Care Coordination: Collaborate with nursing staff, therapists, and specialists to develop and implement individualized, patient-centered care plans.
    • Documentation: Maintain accurate and timely patient records using the PointClickCare (PCC) EMR system.
    • Diagnostics: Utilize and stay current on medical imaging techniques and bedside procedures to improve patient outcomes.
    • Travel: Conduct daily rounds between facilities in Boca Raton area

    Qualifications & Requirements

    • Education: MD or DO degree.
    • Experience: Strong clinical judgment is essential. Experience in geriatrics or SNFs is a plus but not required (training provided).
    • Soft Skills: Exceptional interpersonal and communication skills to effectively engage with patients, families, and facility staff.
    • Travel/Commute: * Willingness to travel locally (approx. 25%).
    • Ability to relocate to the area prior to starting work (Required).
