What You'll Do
- Provide comprehensive primary care, including preventive, acute, and chronic disease management
- Build meaningful, long-term relationships with patients and families
- Collaborate with Whole Health Coaches and care team members to develop personalized care plans
- Use evidence-based medicine and population health strategies to improve patient outcomes
- Empower patients through education, wellness coaching, and shared decision-making
Qualifications
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or a related primary care specialty
- Active Georgia medical license or ability to obtain prior to employment
- DEA registration or ability to obtain
- Passion for patient-centered, relationship-based care
