AS A REMINDER:

• All candidates must have met the minimum shift availability listed on the job.
• CV must have all dates in MM/YY to MM/YY format
• Work History must be in Sequential order with most recent on top
• Any Gaps over 30 days must be explained on the CV
• EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
• All Licenses, Boards and Certs with expiration dates must be listed on CV
• Anyone with malpractice must provide a NPDB
• Anyone with legal issues must provide supporting documents

Availability

• 1st possible date they would be available?
• How many shifts would they be able for per month?
• What month is their schedule completely open?
• Any shift preferences (Day, Night, Blocksetc)?
• Any dates in the next 3-6 months they cant work?
• Any Holidays in the next 3-6 months they would be open to work.?

North Cypress and it's three affiliated Free-Standing ER's. Must be will to cross credential at Free Standings.

• 30,000 annual volumes
• Accredited Chest Pain Centre
• 21 bed ER
• 28 hours of physician coverage

Our Free-standing ERs are all 10-bed ERs, with 24 hour of physician coverage.

SUBMISSION REQUIREMENTS

• Must have TX license and DEA at time of presentation
• Must have clean malpractice/background for temp privs
• Must be Board Certified EM
• Must send self-query NPDB and EPA with presentation
• Must have a recent minimum two years experience in related field
• Credentialing Requirements Physician for all facilities
