About Twin Cities Physicians

Twin Cities Physicians is a provider-led medical group delivering on-site primary care, wound care, and chronic disease management throughout 300+ senior living communities across Minnesota. Our providers serve assisted living, memory care, skilled nursing, and long-term care communities, bringing advanced medical care directly to residents while improving continuity, access, and outcomes.

We don't simply visit communities. We become an extension of their clinical team.

Position Summary

Every senior living community has residents who require more than routine medical management.

The resident with a wound that isn't healing.

The patient whose chronic conditions continue to escalate.

The family looking for answers.

The nursing team seeking clinical guidance.

That's where you come in.

As a Physician Assistant focused on wound care and chronic disease management, you'll become a trusted clinical resource across the communities you serve. Rather than simply treating symptoms, you'll help identify problems early, guide treatment plans, coordinate care, and support better outcomes for medically complex patients.

This role combines primary care, advanced wound management, geriatric medicine, and chronic disease oversight into one highly rewarding opportunity.

• Communicate with families, caregivers, and interdisciplinary teams regarding care plans and treatment goals.
