What You’ll Do:

  • Manage chronic conditions and follow-up care
  • Collaborate with healthcare team to ensure quality patient outcomes
  • Document clinical encounters accurately and thoroughly
  • Support patient education and health promotion activities
What You Need:
  • Valid Physician Assistant or Family Nurse Practitioner license
  • Prior primary care experience (all ages)
  • Comfort with managing mental health concerns
  • Excellent communication skills
  • Ability to work independently and as part of a team
  • No new graduates considered
