    Description

    Stamford Health Medical Group is seeking a driven & self-starter Nurse Practitioner or Physician Assistant to join our well-established and heavily growing Diabetes & Endocrinology Team! In this 100%-based office position, the APRN or PA will closely work alongside our seven Endocrinologists & Diabetes physicians and play a crucial role in providing comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone-related issues to provide a broad spectrum of patient care, including evaluating & educating patients, reviewing labs, triaging patient phone calls, & overseeing medication management.

    Primary Location: 260 Long Ridge Road Stamford, CT with occasional rotation to secondary location

    Work Schedule: Fulltime 40 hours per week; Monday through Friday 8a-5p; no weekends/holidays/call required

    Major Responsibilities:

    • Conduct thorough patient assessments. Approximately 8-12 patients per day.

    • Evaluate and educate patients about diabetes and weight management.

    • Order and interpret diagnostic tests

    • Develop accurate diagnoses and treatment plans

    • Prescribe medication and administer treatments

    • Collaborate with the interdisciplinary team for seamless coordination of care
