Established in 2012 with the creation of a Medicare-certified agency in Southwest Florida, Discovery at Home has expanded to serve patients throughout Florida and Texas. While we primarily serve residents living within Discovery Senior Living communities, our services extend beyond those communities and include a comprehensive range of home health, outpatient, and private duty services.

Responsibilities:

• Assist the Physical Therapist in planning and implementing patient care and therapy services 
• Assist patients with exercises, mobility, and activities of daily living (ADLs) as directed by the PT 

• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed to ensure optimal outcomes

Qualifications:

• Must be a graduate of an accredited Physical Therapy Assistant program
• Must be licensed by the State as a Physical Therapy Assistant

• One year of home health experience preferred
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Background Screening Requirement: Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
