    Physical Therapy Assistant (PTA) | Position Details

    Provides skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist, following the established plan of care. Assists patients in improving mobility, function, and independence while delivering compassionate, patient-centered care.

    Key Responsibilities:

    • Provide physical therapy treatments in the home health setting as delegated by the supervising Physical Therapist.
    • Monitor patient progress, document treatments accurately, and communicate changes in patient status to the supervising therapist.
    • Educate patients, families, and caregivers on exercises, mobility, safety, and home programs to support continued progress.
    • Collaborate with physicians, nurses, and other healthcare team members to coordinate patient care.
    • Maintain timely, accurate documentation in compliance with regulatory, professional, and organizational standards.
    • Participate in quality improvement initiatives and maintain compliance with all applicable policies, procedures, and state practice regulations.
    • Promote positive patient experiences through professional, compassionate, and respectful care.
    • Perform other duties as assigned.

    What You Offer

    • Must be licensed or be eligible for licensure as a PTA in the state of Minnesota
    • Valid driver's license and auto insurance; ability to travel in all weather conditions
    • Strong clinical, communication, and organizational skills
    • Ability to work independently and maintain professionalism and confidentiality
    • Communicate effectively in English, verbally and in writing.
    • Pass a background study through the Minnesota Department of Human Services.

    Work Schedule

    • Day Shift- 8am-4:30pm
    • Casual or Roster Status
