Transitions At Home, is currently seeking to hire a Full-Time Physical Therapist Assistant to make a meaningful impact by helping patients regain independence in the comfort of their own homes.

What does a Transitions at Home Physical Therapist Assistant do? 
An Physical Therapist Assistant (PTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Administrator/Clinical Manager. 
 
Hours: Full-Time
  • 8:00am - 4:30pm 
  • Monday through Friday
  • No Holidays

Qualifications / Job Requirements: 
  • Active Wisconsin Physical Therapist Assistant License 
  • Two (2) years’ experience preferred. 
  • CPR Certification 
  • A valid Wisconsin Driver's License and Car Insurance
