    Physical Therapist Assistant (Part-Time) | Home Health

    Location: Downtown Chicago (Northside), Illinois - Lake View, Lincoln Park, Logan Square, and surrounding areas. 

    About One Home Health

    One Home Health is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our growing home health team on a part-time basis. This role offers the opportunity to provide individualized care to patients in their homes while working alongside an experienced and supportive clinical team.

    Key Responsibilities

    As a Physical Therapist Assistant, you will:

    • Deliver physical therapy treatments and educational support to patients in their homes based on the established physician-approved care plan.
    • Complete required visit documentation accurately and submit records within agency timelines and compliance standards.
    • Observe and report patient responses, progress, concerns, or scheduling changes to the supervising Physical Therapist and care team.
    • Work collaboratively with nurses, therapists, and other healthcare professionals to support coordinated patient care.
    • Participate in interdisciplinary meetings, case reviews, and care planning discussions as needed.
    • Follow all safety procedures, infection prevention practices, and equipment guidelines during patient visits.
    • Safeguard patient privacy and maintain confidentiality in accordance with applicable regulations.
    • Adhere to agency policies and all state, federal, and professional practice requirements.
    • Attend staff development programs, training sessions, and quality improvement initiatives.
    • Perform other related responsibilities as assigned.

    Qualifications

    To be successful in this role, candidates should possess:

    • An active Physical Therapist Assistant license in the state of employment.
    • Graduation from an accredited PTA program recognized by the American Physical Therapy Association (APTA).
    • At least six months of experience working as a Physical Therapist Assistant.
    • Strong communication, documentation, and interpersonal skills.
    • A valid driver's license, dependable vehicle, and current auto insurance meeting agency and state requirements.
    • The ability to travel to patient homes within the designated service area.
