Schedule: Monday–Friday | 8:00 AM–4:30 PM
Location: Fort Wayne, INCoverage Areas: Adams, Allen, Dekalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells, Whitley CountyContract Length: 13+ Weeks Contract (Possibility to Extend)Job Type: Local contractLocal Pay Range: $36.00 - $42.00/hour (W-2) Plus Mileage reimbursementMileage Reimbursement Rate:$0.49/ mile*Pay offered is based on experience, expertise, credentialing, and education. Duties: - Ensures compliance with state-specific laws and regulations governing physical therapy services in home care, following treatment plans established by the supervising Physical Therapist (PT).
- Provides patient instruction and assistance with active/passive exercises, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training within the scope of practice.
- Observes, documents, and reports patient responses to treatment and changes in condition to the supervising PT, nurse supervisor, and/or physician, while recommending plan-of-care changes through the supervising PT.
- Coordinates care with the healthcare team and educates patients, families, caregivers, and team members on therapy-related activities and techniques within the Physical Therapy Assistant's scope of practice.
Requirements: - Education: Bachelor's Degree
- License(s): Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
- Certification(s): Must have current CPR certification
- Years of experience: Minimum 1 –2 years of experience
- Skills/Specialties/Technology: Home Health is preferred
