    About COREWELL HEALTH VNA

    Our Visiting Nurses Association (VNA) is the Home Care Team serving West Michigan. We are seeking a licensed Physical Therapist Assistant to join our growing team supporting patients across West Michigan..

    This role offers a high level of autonomy, allowing clinicians to practice at the top of their license while delivering care in patients’ homes. Home health physical therapy provides a unique opportunity to help patients regain independence and function in the environment where they live and thrive.

    Our therapists maintain manageable caseloads, typically seeing 5–7 patients per day, with thoughtfully designed geographic territories that minimize travel and support work-life balance. As part of Corewell Health West, our VNA team is committed to delivering exceptional, patient-centered care across West Michigan.

    Scope of Work

    Provides rehabilitation therapy procedures to patients under the direction and supervision of a physical therapist. Assists with patients/caregiver education.

    Standard Qualifications

    • Required Associate's Degree accredited physical therapy assistant program
    • LIC-Physical Therapist Assistant - STATE_MI State of Michigan Upon Hire required
    • CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or
    • CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
    • LIC-Driver's License - STATE_MI State of Michigan Home Care Business Lines
