We are seeking a dedicated and experienced Home Health Physical Therapist to join our team. The ideal candidate will possess at least one year of home health experience and will be responsible for providing skilled physical therapy services to our patients in their residences. This is a per-diem position for the areas of Deerfield, Pompano, and Lighthouse Point. 

Key Responsibilities

• Conduct thorough assessments and evaluations of patients' physical abilities and needs in a home setting.
• Develop and implement individualized treatment plans based on patient assessments and clinical findings.
• Utilize various therapeutic techniques, exercises, and modalities to improve patients' mobility, strength and overall function.
• Educate patients and their families on rehabilitation techniques, safety precautions, and home exercise programs.
• Monitor and document patient progress, adjusting treatment plans as necessary
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
