Job Description:-Consults with patients/reviews medical history and referrals on individuals’ physical condition and symptoms; diagnoses movement dysfunction; develops and implements a treatment plan.-Serves as a member of a treatment team; attend care plan meetings, communicates physical therapy treatment plans to family members.-Administers physical therapy treatment programs and services to individuals with more severe or acute physical disabilities.-Assist individuals with the use of equipment such as wheelchairs or walkers. -Teach and coach therapeutic exercise and stretches to improve movement.-Observe and review individuals’ progress and adapt treatment accordingly. -Supervise and train Physical Therapy Assistant, review and cosign paperwork.-Evaluates and initiates skilled Physical Therapy treatments in accordance with provider orders.-Maintains communication with providers.-Performs and completes paperwork and billing in a timely manner.-Participates with interdisciplinary communication and collaboration.-Updates Residents’ care plan when changes are noted.-Participates with discharge planning and home assessments.-Participates with identifying and ordering equipment needed.-Professional interaction with residents, coworkers, visitors, and families.-Attends to responsibilities of the Director of Rehab in their absence.
