| CLIENT: | Phoenix Physical Therapy West End Clinic 1145 19th St. Washington DC |
| SETTING: | PT - OP clinic |
| START DATE: | ASAP - Client will consider any June/July/early Aug start dates |
| GUARANTEE: | 40 hrs/wk |
