Description

Physical Therapist — W-2 Employee:
PCP / Chiro/ PT Practice is a patient-focused healthcare practice committed to providing personalized, high-quality care. We are seeking a compassionate and skilled Physical Therapist to join our team on a part-time basis and help patients improve mobility, function, strength, and overall quality of life.

The Physical Therapist will evaluate and treat patients with a variety of musculoskeletal and functional conditions. The ideal candidate is clinically knowledgeable, patient-centered, and comfortable working collaboratively with physicians and other healthcare professionals.

Responsibilities:
  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
  • Provide therapeutic exercises, manual therapy, neuromuscular re-education, and other appropriate interventions.
  • Monitor patient progress and modify treatment plans as clinically indicated.
  • Educate patients on home exercise programs, injury prevention, and strategies for maintaining functional improvements.
  • Maintain accurate and timely clinical documentation.
  • Communicate effectively with physicians and other members of the healthcare team.
  • Provide a professional, compassionate, and positive patient experience.
  • Follow all applicable clinical, ethical, privacy, and regulatory standards.
  • Submit authorizations for Initial Evals/Re-Evals.

Qualifications:
  • Current New York State Physical Therapist license, or eligibility for licensure.
  • Current CAQH profile.
  • Strong clinical assessment and treatment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while functioning effectively as part of a multidisciplinary team.
  • Commitment to evidence-based, patient-centered care.
  • Prior outpatient or private-practice experience preferred.

Current participation with Medicare and major New York commercial insurance plans is strongly preferred. Please include a list of the insurance plans with which you currently participate. Our credentialing team will assist with adding the selected therapist to Front Street Medical’s group and service locations.

This is a W-2 employee position and is not an independent-contractor assignment.
