Position Summary
The Physical Therapist (PT) will provide skilled rehabilitation services designed to improve mobility, strength, balance, endurance, and overall functional independence for residents in a skilled nursing setting.
Responsibilities
Evaluate and treat residents with mobility impairments, pain, and functional deficits
Develop and implement individualized treatment plans in accordance with physician orders and plans of care
Provide skilled therapy interventions to improve strength, balance, gait, endurance, and mobility
Monitor patient response to treatment and document progress toward goals
Communicate patient progress and recommendations with the interdisciplinary care team
Educate patients, families, and caregivers regarding exercises, mobility, and safety
Maintain accurate and timely documentation in compliance with company policy and state/federal regulations
Collaborate with nursing staff and healthcare professionals to ensure quality patient outcomes
Participate in meetings and care planning as directed by the Director of Rehabilitation
Qualifications
Active and valid Texas Physical Therapist license in good standing
Graduate of an accredited Physical Therapy program
Preferred Qualifications
Skilled nursing facility (SNF) or long-term care experience preferred
Knowledge of Net Health electronic documentation system preferred
Strong communication and interpersonal skills
Ability to work effectively with a diverse patient population
