Position Summary

The Physical Therapist (PT) will provide skilled rehabilitation services designed to improve mobility, strength, balance, endurance, and overall functional independence for residents in a skilled nursing setting.

Responsibilities

  • Evaluate and treat residents with mobility impairments, pain, and functional deficits

  • Develop and implement individualized treatment plans in accordance with physician orders and plans of care

  • Provide skilled therapy interventions to improve strength, balance, gait, endurance, and mobility

  • Monitor patient response to treatment and document progress toward goals

  • Communicate patient progress and recommendations with the interdisciplinary care team

  • Educate patients, families, and caregivers regarding exercises, mobility, and safety

  • Maintain accurate and timely documentation in compliance with company policy and state/federal regulations

  • Collaborate with nursing staff and healthcare professionals to ensure quality patient outcomes

  • Participate in meetings and care planning as directed by the Director of Rehabilitation

Qualifications

  • Active and valid Texas Physical Therapist license in good standing

  • Graduate of an accredited Physical Therapy program

Preferred Qualifications

  • Skilled nursing facility (SNF) or long-term care experience preferred

  • Knowledge of Net Health electronic documentation system preferred

  • Strong communication and interpersonal skills

  • Ability to work effectively with a diverse patient population
