    What You'll Do:

    • Conduct assessments both in facility and occasionally at home to guide care plans
    • Participate actively in team-based care planning discussions
    • Deliver personalized physical therapy interventions tailored to each participant
    • Provide training to caregivers and supervise therapy assistants
    • Promote safety, mobility, and independence in a holistic, supportive setting

    What You’ll Need:

    • Bachelor's degree in Physical Therapy (Master’s or Doctorate preferred)
    • Valid California Physical Therapy License (or eligibility)
    • 1-2+ years of experience in outpatient, acute, inpatient, or SNF settings preferred
    • CPR certification preferred
    • Passion for serving frail or elderly populations with compassion and respect
