Physical Therapist – Home Health

Are you a Physical Therapist looking for more flexibility, independence, and earning potential without being stuck in a clinic all day?

A growing Home Health Agency is seeking a Full-Time Physical Therapist to provide one-on-one care to patients throughout the Port Charlotte and Punta Gorda area.

This position offers a competitive $95,000–$115,000 annual compensation package, a manageable territory, flexible scheduling, and full benefits.

What You'll Do

• Provide skilled Physical Therapy services to patients in their homes

• Perform evaluations and develop individualized plans of care

• Help patients improve mobility, strength, balance, and functional independence

• Complete reassessments and discharge evaluations as appropriate

• Educate patients, families, and caregivers on exercises and safety

• Coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team

• Complete documentation accurately and timely

• Manage your daily patient schedule and territory efficiently

What We're Looking For

• Current Florida Physical Therapist license

• Graduate of an accredited Physical Therapy program

• Strong clinical and communication skills

• Ability to work independently in a home-based environment

• Reliable transportation and valid driver's license

• Home Health experience is preferred, but qualified PTs from other settings are encouraged to apply

• OASIS experience is a plus
