Job Title: Physical Therapist (PT) – Home Health (Contract)

Location: Salem, AR (Coverage within 50-mile radius)
Employment Type: Contract
Duration: 13 Weeks
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Hours: 32–40 hours per week (flexible)

Position Overview

We are seeking a licensed Physical Therapist (PT) to provide high-quality home health care services in and around the Salem, AR area. This role focuses on patient evaluations, reassessments, and start-of-care visits, ensuring timely and effective treatment in a home setting.

Key Responsibilities

• Conduct Physical Therapy evaluations and reassessments
• Perform Start of Care (SOC) visits for home health patients
• Provide treatment visits as needed
• Deliver high-quality patient care in a home health environment
• Ensure timely and accurate documentation (within 24 hours)
• Collaborate with interdisciplinary teams to support patient outcomes

Required Qualifications

• Active Physical Therapist (PT) license in the state of Arkansas
• CPR Certification
• Prior experience in home health settings
• Experience using Homecare Homebase (HCHB)

Work Schedule & Coverage

• Schedule: Monday–Friday, 8:00 AM – 4:30 PM

• On-Call/Weekend Requirement: None

Productivity & Expectations

• Target productivity: 24–30 points per week
• Timely documentation submission (within 24 hours)
