Job Description:
We are seeking a committed Physical Therapist to join a skilled nursing and post-acute rehabilitation center in Sacramento, CA. The facility serves adults and seniors recovering from hospitalization, surgery, and complex medical conditions. You will conduct evaluations, develop evidence-based treatment plans, and provide interventions focused on gait, balance, strength, and functional independence. The interdisciplinary care team values communication, shared care planning, and quality outcomes. This role offers a full-time schedule and a clinically supportive environment.

Responsibilities

  • Perform comprehensive physical therapy evaluations in a SNF/post-acute setting.

  • Develop and implement individualized treatment plans based on patient goals.

  • Deliver therapeutic exercises, gait training, and balance retraining.

  • Reassess patients and modify plans of care as conditions change.

  • Educate patients and caregivers on safe mobility and home programs.

  • Document all care in accordance with regulatory and facility standards.

  • Collaborate with nursing, physicians, and other disciplines to coordinate care.

Experience / Qualifications

  • Valid state license as a Physical Therapist.

  • SNF, inpatient rehab, or post-acute experience preferred.

  • Strong clinical assessment and treatment-planning skills.

  • Ability to manage a varied caseload and prioritize effectively.
