Responsibilities

• Assess and treat patients to improve mobility and reduce pain in a clinical setting.

• Provide patient-first care while upholding ethical and regulatory standards.

• Create individualized rehabilitation plans with clear milestones and timelines.

• Monitor patient progress and adjust therapy plans accordingly.

• Document treatment sessions and outcomes in electronic medical records.

• Educate patients and families on exercises, precautions, and home programs.

• Collaborate with multidisciplinary team to coordinate care and optimize outcomes.

• Maintain on-site clinic readiness and equipment safety/sterilization standards.

• Meet or exceed quarterly targets and contribute to pipeline-based planning.

• Demonstrate professional growth by pursuing continuing education opportunities.

• Engage in regular performance reviews and reflect on personal development goals.

Experience/Qualifications

• Licensed physical therapist with active state license and CPR certification.

• Minimum 2 years of clinical physical therapy experience in rehab settings.

• Proficiency in evaluating mobility, pain, and functional goals with measurable milestones.

• Ability to create individualized rehab plans and adjust based on progress data.

• Strong documentation skills with electronic medical records and outcome tracking.

• Experience in patient and family education on home programs and precautions.

• Collaboration with multidisciplinary team and care coordination for optimal outcomes.

• Performance-driven mindset with track record of meeting or exceeding targets.

• Commitment to clinic readiness, equipment safety, and sterilization standards.

• Demonstrated commitment to continuing education and professional growth.
