Physical Therapist (PT) visit in Brockton/ Stoughton/ Taunton/ Plymouth, MA

Job Summary: Provide skilled physical therapy services to patients in their homes according to physician orders and the established plan of care. Perform comprehensive assessments, develop and implement treatment plans, monitor patient progress, document care accurately, educate patients and caregivers, and prepare patients for discharge when appropriate.

Qualifications/Educational Requirements:  

• 1- Current Physical Therapist license in the state of practice (if required by state law).
• 2- Graduate of a CAPTE-accredited Physical Therapy program or an equivalent international program recognized by an approved credentialing organization.
• 3- Successfully passed the state-approved Physical Therapist licensing examination.
• 4- Must meet applicable federal and state qualification requirements for Physical Therapists, including legacy qualification pathways for those licensed or trained before current standards.

  Responsibilities/essential functions:

  
  The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

   

  1. Provide skilled physical therapy according to the physician's plan of care
  2. Evaluate patients using appropriate assessment tools and assist in developing and updating individualized care plans.
  3. Deliver therapeutic treatments, modalities, and interventions to reduce pain, restore function, improve mobility, and maximize independence.
  4. Monitor, document, and report patient progress, treatment responses, and changes in condition to the physician, Clinical Manager, and Case Manager.
  5. Educate patients, caregivers, and healthcare team members on therapy techniques, home exercises, assistive devices, prosthetics, orthotics, and durable medical equipment as appropriate.
  6. Regularly reassess patient needs and revise treatment plans as necessary.
  7. Initiate appropriate diagnostic, preventive, and rehabilitative interventions to promote patient safety and recovery.
  8. Coordinate care with physicians and interdisciplinary team members through communication and case conferences.
  9. Maintain accurate, timely, and compliant clinical documentation.
  10. Participate in quality improvement initiatives, utilization reviews, staff meetings, and continuing education.
  11. Stay current with physical therapy best practices, documentation requirements, and regulatory standards.
  12. Provide staff education as directed by the Clinical Manager.
  13. Supervise Physical Therapy Aides at least every 30 days, when applicable.

  Environmental Elements
  
  Employee works in an office environment sometimes with moderate noise levels, controlled temperature conditions and sometimes travels to patients homes where they may have direct exposure to hazardous substances. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures.
  
  Employee will have to travel to a variety of patient homes and perform in conditions that vary greatly depending upon the client’s home environment. Some homes will be clean, neat, and maintained at a comfortable temperature. Other homes may be cluttered, dirty, with an uncomfortable temperature.
