Schedule: PRN | Minimum 3 shifts per month
Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist, you’ll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections. Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you’ll play a key role in helping clients achieve their highest level of independence and well-being.
As a Physical Therapist, you will:
- Deliver Direct Client Care – Conduct comprehensive evaluations and develop personalized treatment plans with short-term and long-term goals.
- Maintain Accurate Client Records – Document services accurately and timely using Vivie’s EMR system, ensuring compliance with all payer sources and authorization requirements.
- Facilitate Follow-Up Programs – Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes.
- Collaborate and Communicate Effectively – Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
- Ensure Regulatory Compliance – Complete and document supervisory visits with PTAs and RA/CNAs in accordance with state and regulatory guidelines.

This job also requires:
- Current Physical Therapist license in the state of Minnesota.
- Strong collaboration and teamwork skills.
- Clear and compassionate communication abilities.
- Proficient in English for effective communication with staff, clients, and visitors.
- Successful completion of a state-mandated background check.
- Able to physically perform the duties required by the position.
- Must obtain CPR certification within 6 months of hire.
