    Concept Rehab is hiring a Physical Therapist (PT) to join our therapy team in Johnston, Iowa. We are currently offering Full-Time, Part-Time, and PRN opportunities at our skilled nursing facility.     About the Role


    As a Physical Therapist, you will evaluate and treat patients recovering from illness, injury, surgery, and hospitalization in a skilled nursing and post-acute rehabilitation setting.     Qualifications



    • Graduate of an accredited Physical Therapy program

    • Current Iowa Physical Therapist license (or eligible for licensure)

    • Excellent communication and interpersonal skills

    Key Responsibilities



    • Evaluate patients within 24–48 hours of physician orders

    • Develop and implement individualized treatment plans

    • Provide skilled, patient-centered therapy interventions

    • Supervise and collaborate with Physical Therapy Assistants

    • Communicate effectively with physicians, nursing, families, and care teams

    • Participate in interdisciplinary and care planning meetings

    • Document evaluations, treatments, and progress accurately and timely

    • Support discharge planning and caregiver education

    • Participate in staff education and in-service training as appropriate

    Qualifications



    • Bachelor’s, Master’s, or Doctorate in Physical Therapy

    • Active Iowa Physical Therapist license
