Compensation:

• $58 – $70 per hour

Position Summary & Responsibilities (SNF)

• Evaluate and treat SNF residents

• Deliver skilled therapy and document care

• Ensure Medicare/PDPM compliance

• Collaborate with interdisciplinary team

• Maintain productivity and quality standards

Requirements

• Active Physical Therapist (PT) license

• SNF or post-acute rehab experience required

• Knowledge of Medicare/PDPM and clinical documentation standards

• Strong communication and teamwork skills
