Duties and Responsibilities:
- Perform initial and ongoing clinical assessments to identify patient needs.
- Develop and revise a Plan of Care in consultation with the physician and care team.
- Provide therapeutic treatments and evaluate equipment needs.
- Evaluate outcomes of care with appropriate discharge planning.
- Apply infection control concepts to prevent contamination.
- Supervise PTA and HHA as appropriate.
- Participate in mandatory continuing education and in-service programs.
- Wound care management.
Required Skills/Abilities:
- Strong therapy skills based on accepted standards of practice.
- Excellent interpersonal skills.
- Knowledge of durable medical equipment.
- Ability to work a flexible schedule and travel locally.
- Reliable transportation and valid auto liability insurance.
Required Education and Experience:
- Bachelor of Science in Physical Therapy from an accredited program.
- Current Texas State License as a Physical Therapist.
- Two years' experience in an acute care or rehabilitation setting.
Physical Requirements:
- Must be able to lift and/or move up to 50 pounds.
- Frequent talking, standing, and hand dexterity required.
- Close vision, color vision, and depth perception abilities needed.
Work Location & Travel Requirements:
- Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA
- This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
