Pride Health is actively hiring a Travel/local - Physical Therapist (PT) to join our client's Home Health Team.
 
Job Details: 
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Location:  Tallahassee, FL, 32308
Float / Coverage Area: Leon, Wakulla, Madison, Jefferson, and Taylor Counties
Job Type: Local Contract
Contract Length: 14 Weeks (Possible Extension)
Pay Range: $50.00 to $57.00/hour*
Mileage Reimbursement Rate (if applicable): $0.53/ mile
*Pay offered is based on experience, expertise, credentialing, and education.  
 
Duties: 

• Perform comprehensive physical therapy evaluations and determine patient eligibility for therapy services.
• Develop, implement, and revise individualized Plans of Care based on physician referrals, patient assessments, and clinical goals.
• Provide therapeutic interventions for orthopedic, neurological, and neuromuscular conditions.
• Improve patient mobility, strength, balance, endurance, safety, and functional independence.
• Educate patients and caregivers regarding therapeutic exercises, home safety, fall prevention, and adaptive equipment use.
• Complete accurate and timely clinical documentation, including required OASIS assessments and progress notes.
• Coordinate care with physicians, nurses, caregivers, and interdisciplinary healthcare team members.
• Ensure compliance with Medicare, CMS, agency, and regulatory requirements.

Requirements: 

• Active Physical Therapist (PT) License.
• Current CPR Certification.
• Valid Driver's License, Reliable Transportation, and Auto Insurance.
• Strong patient assessment and treatment planning skills.
