Key Responsibilities
Patient Care & Treatment
Perform initial and ongoing physical therapy evaluations
Develop and implement individualized plan of care (POC) based on patient needs
Provide therapeutic exercises, manual therapy, and functional training
Assess patient mobility, strength, balance, and coordination
Modify treatment plans based on patient progress and outcomes
Care Coordination
Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals
Communicate patient progress and clinical findings to the care team
Participate in case conferences and interdisciplinary meetings
Educate patients and caregivers on home exercise programs and safety
Documentation & Compliance
Complete accurate and timely clinical documentation in the EHR system
Ensure compliance with Medicare Conditions of Participation (CoPs) and agency policies
Maintain proper documentation for evaluations, progress notes, and discharge summaries
Follow all state and federal regulations related to home health services
Patient & Family Education
Educate patients and families on treatment plans, exercises, and fall prevention
Promote independence and safety within the home environment
Provide recommendations for assistive devices and home modifications
Qualifications
Active Physical Therapist (PT) license in Nevada
Graduate of an accredited Physical Therapy program
Minimum 1 year of clinical experience (home health preferred)
Strong knowledge of rehabilitation techniques and patient care standards
Excellent communication and interpersonal skills
Ability to work independently and manage time effectively
Preferred Qualifications
Home health experience strongly preferred
Experience with OASIS documentation
Familiarity with home health EMR systems (e.g., Axxess, WellSky)
Bilingual (English/Spanish) is a plus
Physical Requirements
Ability to travel to patient homes within assigned territory
Ability to lift, assist, and support patients as needed
Ability to perform physical tasks such as standing, walking, bending, and reaching
Work Environment
Field-based position providing care in patients’ homes
Flexible scheduling depending on patient caseload
Independent work with support from clinical leadership
