YOUR PRIMARY RESPONSIBILITIES WILL INCLUDE:

• Conducting thorough physical assessments and reviewing medical histories to evaluate patients' conditions.
• Creating individualized treatment plans tailored to each patient's specific needs and rehabilitation goals.
• Implementing therapeutic exercises and training programs to enhance mobility and functionality.
• Educating patients and their families about injury prevention, wellness, and recovery processes.
• Monitoring and documenting progress towards rehabilitation objectives, adjusting treatments as necessary.
• Collaborating with other healthcare professionals to ensure comprehensive care.
• Maintaining precise patient records and documentation in accordance with regulatory standards.

QULIFICATIONS:

• Masters degree in Physical Therapy.
• A minimum of 1 year of experience in home health as a physical therapist.
• Valid state licensure to practice as a physical therapist, adhering to professional standards and practices.
• Current CPR certification required during clinical employment.
• Valid driver's license with the ability to travel to various client locations.
