Start Date: 03/01/2026 (Transitioning in-house from Consonus)
Schedule: Permanent or Travel

Compensation:

• $62 – $72 per hour

Position Summary & Responsibilities (SNF)

• Evaluate and treat SNF residents

• Document care; ensure Medicare/PDPM compliance

• Collaborate with interdisciplinary team

• Maintain productivity and quality standards

Requirements

• Active Physical Therapist (PT) license

• SNF or post-acute rehab experience required

• Knowledge of Medicare/PDPM and documentation standards

• Ability to adapt during transition to in-house therapy model
