    Job Summary

    Empath Home Health, a member of Empath Health, is seeking highly motivated and compassionate Home Health Physical Therapists (PTs) to join our Florida team.

    As part of our Physical Therapy Team, you will work alongside a dynamic clinical team dedicated to helping patients return to and maintain their highest level of physical functioning.     Key Responsibilities

    • Provide skilled physical therapy services to patients as prescribed by a physician while following agency policies, procedures, and applicable federal and state regulations.
    • Perform initial and ongoing patient assessments, including OASIS assessments at appropriate time points.
    • Evaluate patients’ functional abilities and their ability to perform activities of daily living (ADLs).
    • Develop, implement, and revise individualized plans of care in collaboration with physicians and other members of the care team.
    • Establish realistic mobility goals and provide therapeutic interventions to help patients achieve optimal outcomes.
    • Monitor patient progress and response to treatment.
    • Report patient reactions to treatment and changes in condition to the physician and designated agency representatives.
    • Maintain accurate and timely documentation according to organizational standards and regulatory requirements.
    • Educate patients and caregivers on treatment plans, mobility strategies, and safety techniques.

    Required Qualifications

    One to two (1–2) years of Physical Therapist experience

    Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program

    Active Florida Physical Therapist License

    Current BLS Certification
    (American Red Cross or American Heart Association)


    Preferred Qualifications

    Minimum of one (1) year of Home Health Physical Therapy experience

    Experience with OASIS assessments

    Strong clinical assessment, documentation, and patient education skills


    Schedule & Compensation

    Full-Time Position

    40 Hours per Week

    Day Shift

    Some weekends required
    (Schedule details will be discussed during the interview process.)
