We are looking for motivated and compassionate Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide quality, patient-centered care in a home health setting. Our team works closely with patients, families, physicians, and other healthcare professionals to help patients improve mobility, strength, independence, and overall quality of life.


Job Summary
The Physical Therapist (PT) evaluates patients, develops and updates individualized plans of care, provides skilled physical therapy treatments, evaluates patient progress and outcomes, and collaborates with physicians and the interdisciplinary care team. The Physical Therapist Assistant (PTA) provides direct physical therapy services under the direction and supervision of a licensed PT, follows the established plan of care, monitors patient progress, and provides education to patients and caregivers. Both positions are responsible for providing safe, effective, and compassionate care in the patient's home.


Essential Duties & Responsibilities
Physical Therapist (PT):
  • Perform initial and ongoing assessments to identify patient needs, functional limitations, and appropriateness for home health services.
  • Develop and revise individualized Plans of Care in consultation with the physician and interdisciplinary care team.
  • Provide skilled physical therapy evaluations and therapeutic treatments to improve mobility, strength, function, and independence.
  • Evaluate patient outcomes and make appropriate recommendations for continued care or discharge.
  • Assess equipment and durable medical equipment needs as appropriate.
  • Supervise PTAs and HHAs as applicable.
  • Monitor patient response to treatment and modify interventions as appropriate.
  • Complete all required clinical documentation accurately and in a timely manner.
  • Communicate effectively with patients, caregivers, physicians, and other members of the healthcare team.
  • Follow infection control, safety, and universal precaution procedures.
  • Participate in required continuing education, in-service training, and professional development.
  • Promote the agency's philosophy, policies, and standards of care.
Physical Therapist Assistant (PTA):
  • Provide direct patient care under the direction and supervision of a licensed Physical Therapist.
  • Provide services within the scope of practice and consistent with education and clinical competence.
  • Carry out treatments and interventions as ordered and outlined in the patient's Plan of Care.
  • Perform therapeutic exercises and appropriate physical therapy modalities within the scope of practice.
  • Monitor and document patient progress, functional changes, and response to treatment.
  • Educate patients and caregivers regarding treatment and appropriate home activities.
  • Complete required visit notes, clinical documentation, and other records accurately and promptly.
  • Communicate effectively with patients, caregivers, physicians, the supervising PT, and other healthcare team members.
  • Follow infection control, safety, and universal precaution procedures.
  • Participate in required continuing education and agency-sponsored in-service training.
  • Ensure compliance with applicable payor requirements.
  • Perform other duties as assigned.
Qualifications
Physical Therapist (PT):
  • Current, active Texas Physical Therapist license.
  • Current Texas driver's license.
  • Current CPR certification as required for home visits.
  • Reliable transportation and valid auto liability insurance.
  • Home health experience preferred.
  • Bilingual Spanish preferred.
Physical Therapist Assistant (PTA):
  • Current, active Texas Physical Therapist Assistant license.
  • Current CPR certification.
  • Current Texas driver's license.
  • Reliable transportation and valid auto liability insurance.
  • Home health experience preferred.
  • Bilingual Spanish preferred.
Physical & Work Environment
  • Ability to travel locally to patients' homes.
  • Ability to work in a variety of home and healthcare environments.
  • Ability to stand and walk for extended periods.
  • Ability to bend, push, pull, and perform physical activities associated with patient care.
  • Ability to work flexible schedules based on patient needs.
  • Possible exposure to blood, body fluids, infectious diseases, and varying weather conditions.
