    Description

    A facility in Cheraw, SC is seeking a Physical Therapist to join its outpatient rehabilitation department. This role focuses on providing direct therapy services that support patient recovery, mobility improvement, and long-term functional independence in an outpatient clinical setting.

    Position Overview

    The Physical Therapist will assess patients, develop individualized treatment plans, and deliver evidence-based rehabilitation services. Care is provided in collaboration with other clinical team members to ensure coordinated, patient-centered outcomes across the continuum of outpatient rehabilitation services.

    The role requires strong clinical reasoning, effective communication, and the ability to manage a varied caseload while working independently within established clinical guidelines.

    Key Responsibilities

    • Conduct outpatient physical therapy evaluations and assessments
    • Develop and implement individualized treatment plans based on patient needs
    • Deliver therapeutic interventions to improve strength, mobility, and function
    • Monitor and document patient progress and treatment outcomes
    • Collaborate with physicians and clinical staff to coordinate care plans
    • Educate patients on home exercise programs and recovery strategies
    • Maintain compliance with clinical protocols and safety standards
    • Adjust treatment plans based on patient response and progress
    • Support continuity of care through effective documentation and communication

    Qualifications

    • Graduate of an accredited Physical Therapy program required
    • Active and unrestricted Physical Therapy license in South Carolina required
    • Current CPR certification required
    • Previous outpatient or rehabilitation experience preferred
    • Strong clinical judgment and communication skills required
