Job Title: Physical Therapist - Outpatient Rehab Clinical CoordinatorDepartment: Outpatient TherapySupervisor: V.P. Community Services and Business Development Job Objective:This position provides clinical and operational leadership for Mozaic Senior Life's outpatient rehabilitation program while maintaining an active patient treatment caseload. Approximately 75–80% of time to direct patient care and 20–25% to coordination and leadership responsibilities. This a key role responsible for supporting high-quality, patient-centered therapy services that support wellness, recovery, functional independence, and aging in place as well as daily clinic operations, operational oversight staff coordination, program growth, and community engagement. Physical and Mental Requirements:Ability to sit or stand for long periods of time (often 6-8 hours per day).Ability to lift, transfer, and reposition patients (typically up to 50 lbs. independently; more with assistance or equipment)Frequent walking, bending, stooping, kneeling, crouching, and reaching. Ability to perform manual therapy techniques requiring upper body strength and coordination as well as hand/eye coordination such as keyboard skills, writing or picking up paper.Ability to communicate clearly and compassionately while maintaining professionalism and empathy in emotionally demanding situations.Ability to motivate people who may be reluctant, fatigued, declining or who have cognitive issues.Ability to maintain focus with some interruptions while supervising clients and preventing falls and injuries during mobility tasks. Able to hear sounds within some background of noise as in answering the phone, etc.Visual acuity to see clients, a computer and read printed documents.Ability to use company software and equipment to accomplish electronic tasks.Ability to stay focused on tasks for more than 60 minutes at a time.Ability to understand and relate to the reasons behind several related concepts.Ability to remember multiple tasks /assignments given to self and others over long periods of time.
